Skilled Nursing Services
RVNA's nursing staff will provide you with professional care in accordance with the treatment plan prescribed by your physician for:
- Chronic disease management
- IV therapy, blood draws
- Post-operative care

- Wound care, dressing changes
Rehabilitation Therapy
RVNA Physical and Occupational Therapists work with patients to improve:

Our Speech Therapists help patients overcome difficulties in:
- Language/articulation

Home Health Aide
RVNA Home Health Aides provide home care services to meet day-to-day personal care needs of patients, enabling them to remain in independent living situations.

Medical Social Worker
RVNA's Medical Social Worker will assist individuals with:
- Social services
- Facilitate community resources
- Family support
RVNA Health Buddy®
RVNA patients with telehealth monitoring have shown a measurable reduction in incidence of re-hospitalization for patients with chronic disease. Health Buddy® services will help patients with COPD, CHF, diabetes or hypertension. RVNA's telehealth system is an in-home device used daily by patients to help them manage their disease through monitoring and disease education. The unit will record weight, glucose levels, blood pressures, pulse oximetry levels and prompt patients to comply with the medication regimen.
The Health Buddy® telehealth system:
- Is monitored daily by a nurse, allowing early identifications and timely intervention if needed.
- Asks educational questions that help patients to better understand and manage their chronic disease.
- Produces a greater communication on behalf of the patient between the nurse and physician through shared reports and patient data.

To fulfill this mission, we will provide preventive, healing, rehabilitative, palliative and end of life services through a collaborative system of professionals and paraprofessionals; preserve the dignity of each individual regardless of creed, race, sex, age, disability, or social status; maintain standards of excellence in all aspects of patient care and operate in a cost-effective and financially responsible manner.

Since 1979, Rehabilitation and Visiting Nurse Association has depended on community support to aid their mission of home health care to people of all ages. Donations allow us to be sensitive to patients ability to pay, and support current programs, as well as the development of new health programs, and help purchase supplies and equipment.
