Rehabilitation and Visiting Nurse Association (RVNA) is northern Colorado’s only non-profit, community-based home health care agency, serving Weld and Larimer Counties for over 30 years.

We provide exceptional in-home health care that includes skilled nursing, physical therapy, occupational therapy, speech therapy, personal care and homemaking assistance. Our clients come from all walks of life and all age groups. Many receive home health care based on physician’s orders as they recuperate from an illness, condition, or surgery, but others receive RVNA services based on their own desire to receive assistance to complete activities of daily living and maintain independence in their own home. We accept Medicare, Medicaid, Private Insurance and Private Pay.

  • Stay Well® Solutions. Even when caregivers are not present, RVNA can help you stay safe and healthy using contemporary technology and monitors.
  • Health Buddy, a monitor by phone for a variety of health factors
  • Med-Time, a personal medication dosing and timing system
  • VoiceCare, a personal emergency response system

Accreditation & Affiliations

Rehabilitation and Visiting Nurse Association has achieved accreditation from Community Health Accreditation Program. (CHAP).

CHAP is dedicated to improving the quality of the national health care through voluntary accreditation. RVNA, which provides a number of services, received the accreditation award after CHAP found that it had demonstrated compliance to CHAP's national standards for home care organizations. Achieving accreditation demonstrates that RVNA is committed to provide high quality care to its patients.

RVNA is a member of the National Association of Home Care (NAHC), the Visiting Nurse Association of America (VNAA) and the Home Care Association of Colorado. (HCAC). Rehabilitation and Visiting Nurse Association is a United Way affiliated agency.
